We are looking for the CMS of Avenches at a rate between 50%-60%, a

ASSC

Mission:

• Collaborate on the medical-social support project (accompaniment in the form of aid and care) based on respect for the principle of autonomy of the person.
• Participate in monitoring the evolution of the client's situation, in collaboration with the client, the entourage, the interdisciplinary team, and the treating physicians.
• Provide clients with basic care services, nursing care, household help, and family assistance.

Profile:

• Have a CFC in ASSC.
• Have, if possible, experience in home care.
• Be available to work during the day, evening, and weekends.
• Be able to work independently and in an interdisciplinary manner.
• Hold a driver's license and own a vehicle (possibility of using a professional vehicle).
• Knowledge of German would be an asset.
