Exciting challenges await you ###

• Counseling, care, and support of patients and their relatives in sometimes difficult life situations
• Mediation and organization of medically prescribed rehabilitation and spa stays
• Support, counseling, and organization of transfers to elderly and nursing homes
• Counseling and support in the planning of outpatient care at home (e.g., Spitex, meal services, etc.)
• Mediation and counseling regarding facilities and services in the areas of social assistance, addiction assistance, and disability facilities
• Coordination with relevant external specialist agencies, authorities, and municipalities
• Participation in interdisciplinary reports, rounds, working groups, and projects
• Planning and organization of patient transports
• Recording patient statistics within your own area of responsibility

What distinguishes you for the job ###

• specific education and further training in the field of social work/healthcare; relevant professional experience in a comparable institution of the healthcare or social sector is an advantage
• good knowledge of relevant issues in health, accident, and social insurance law
• empathetic, balanced, and resilient personality with strong social skills
• good oral and written communication skills; stylistically confident in German, both spoken and written
• independent, responsible, and structured way of working
• good administration and IT skills
