We are looking for

Nursing Specialist Bedarfsabklärung

Your tasks:

• Bedarfsabklärung: Conducting needs assessments with clients at home to determine care needs and required services.
• InterRAI HC-Assessments: Applying the InterRAI HC system to conduct standardized needs assessments, ensuring precise care and support planning.
• Consulting: Advising clients and their relatives on care and support services.
• Creating care plans: Developing individual care and support plans in cooperation with nursing specialists and the interdisciplinary team.
• Coordination: Closely collaborating with caregivers, doctors, Spitex employees, and other specialists to ensure seamless care.

Documentation:

• Good knowledge in creating care plans and needs assessments.
• Strong communication and empathy skills when dealing with clients and their relatives.
• Independent, structured, and responsible work approach.
• Careful and timely documentation of needs assessments and care planning in the system, particularly InterRAI HC data.
• Quality control: Contributing to ensuring high-quality standards in care and implementing developments in the area of needs assessment.
• Close collaboration with the Deputy Nursing and Care/Pflegeexperte/in

Your profile:

• Diplomate in nursing specialist HF or FH (or comparable training in healthcare). RAI expert, InterRAI HC
• Experience in Spitex: Extensive experience in caring for and supporting clients in their home environment, particularly in Spitex.
• Knowledge: Experience in applying the InterRAI HC system for needs assessment and care planning, or willingness to learn this system.
• Interest and ability to conduct needs assessments independently and in a solution-oriented manner.
• Very good communication skills and empathy when dealing with clients and their relatives.
• Independent, responsible work approach, as well as high flexibility and resilience.
• Very good IT skills and experience with documentation systems. MS Office, nursing documentation systems, Perigon experience an advantage
