Nursing Expert / Nurse Practitioner USZ@Home (f/m/d) 60-80%

In cooperation with a second Nurse Practitioner (NP), you will care for patients with the USZ@Home team who are being continuously cared for at home on an outpatient basis after discharge. In the development phase, these are patients with IV therapies, wound dressings, or pain. The Universitätsspital Zürich is one of the largest hospitals in Switzerland. Our over 8'600 employees and around 700 trainees are committed every day to the well-being of our patients. ### What tasks await you? • Participation in the development and implementation of the new USZ@Home care model, e.g., elaboration of treatment pathways with the involved clinics, creation & implementation of guidelines for the care of USZ@Home patients, establishment of a stable cooperation with external partners such as Spitex and general practitioner services • Assessment and inclusion of suitable patients for USZ@Home care • Performance of telemedicine, interprofessional rounds • Management of discharge management from the USZ@Home setting • Coordination of various internal & external services • Coaching & empowerment of internal and external employees for the implementation of the USZ@Home treatment pathways • Further development of the USZ@Home care model together with the project team for scaling for further groups of patients Who are we looking for? • A certified nursing professional with several years of professional experience and a completed Master of Science in Nursing. • A person with an interest in merging research & practice in everyday professional life and who, with commitment and joy, dedicates themselves to the further development of the USZ@Home care model with us. • A person with high personal responsibility, who moves skillfully in the interprofessional setting and enthusiastically empowers both patients and employees.
